Professional REFERRALS

A Clear Path to Dependable In-Home Support

Shirley’s Angels Home Care accepts referrals for seniors and individuals with disabilities who may need compassionate, non-medical support at home or in the community. Our intake team works with individuals, families, guardians, case managers, care coordinators, and referral partners throughout Indiana.

Submit the secure referral form below, and our team will review the information provided, confirm the requested services and payer details, and communicate the next steps.

What Happens After a Referral Is Submitted

Our intake process is designed to make the next steps clear while allowing time to review eligibility, authorization, care needs, scheduling, and caregiver availability.

01

Submit a Referral

Complete the secure referral form and provide the client’s contact information, requested services, payer details, location, and known scheduling needs.

03

Authorization and Service Confirmation

For Medicaid-funded services, we confirm the service authorization and coordinate with the case manager, care coordinator, payer, or managed-care partner when needed. Private-pay arrangements are discussed directly with the responsible party.

02

Intake Review and Consultation

Our intake team reviews the referral and contacts the appropriate person to clarify service needs, care-plan information, preferred schedule, and desired start date.

04

Intake Completion and Staffing Review

After required documents and authorizations are received, we complete the intake process and review caregiver availability for the requested location and schedule.

“A strong referral gives our team the information needed to move the intake process forward clearly and efficiently.”

Submitting a referral does not guarantee eligibility, authorization, staffing, or an immediate start date. Service availability depends on the approved care plan, requested schedule, location, intake completion, and available caregivers.

Begin a Referral

Share a few details about the person needing services. Our intake team will review the information and follow up promptly.
