Our intake team works with individuals, families, guardians, case managers, care coordinators, and referral partners throughout Indiana.

Complete the secure referral form and provide the client’s contact information, requested services, payer details, location, and known scheduling needs.

Authorization and Service Confirmation
For Medicaid-funded services, we confirm the service authorization and coordinate with the case manager, care coordinator, payer, or managed-care partner when needed. Private-pay arrangements are discussed directly with the responsible party.

Intake Review and Consultation
Our intake team reviews the referral and contacts the appropriate person to clarify service needs, care-plan information, preferred schedule, and desired start date.

Intake Completion and Staffing Review
After required documents and authorizations are received, we complete the intake process and review caregiver availability for the requested location and schedule.
